They are also giving total strangers a copy of their I.D. and birth information. These people can then apply for a birth certificate and steal your identity. Please understand that our office is not associated with any company that allows you to apply for a birth certificate online. Our fee is only $10.00. If you would prefer to use a credit card please contact the Indiana State Department of Health at vitalrecords.in.gov You will be able to order your certificate online with a credit card. Print a Birth Certificate Application Print a Death Certificate Application You can not request a birth certificate or death certificate by phone or email. If you are including an over-night envelope with your request please be sure you have paid the company any additional fees they may charge for picking it up.
